Pharmacist (Clinical Pharmacy Specialist)

The Clinical Pharmacy Specialist (CPS) serves as a consultant and mid-level provider with Telehealth Service. The CPS functions at the highest level of clinical practice, working independently under his/her scope of practice as defined by the medical center to directly care for patients at the parent facility, affiliated CBOCs, other areas of the medical center, or other facilities or CBOCs consulted for care through telehealth or other virtual modalities. This CPS functions to initiate, modify or discontinue medication therapy with an emphasis on the management of diabetes, hypertension, hyperlipidemia, anticoagulation, smoking cessation, and naloxone administration. Designs, implements, assesses, monitors and documents therapeutic plans utilizing the most effective, least toxic, and most economical medication treatment for patients consulted to her/his care and other clinics as designated by supervisor. Works independently and effectively to evaluate prescriptions for accuracy, eligibility, availability, and cost effectiveness and therapeutic appropriateness; contacts physicians and makes authorized changes in orders or prescriptions as required. Functions independently in day-to-day clinical and administrative duties associated with this position, consulting with higher level supervisors only when required. In general, uses a high degree of appropriate professional judgment to solve complex problems which routinely arise throughout the work day. When required, communicates any problems to the medical staff and/or other health care professionals, and offers alternatives or solutions to the problems. Demonstrates recognized expertise in the assigned area of practice for the purpose of chronic disease state management, including-but not limited to-thromboembolism, diabetes, hypertension, hyperlipidemia, smoking cessation, and naloxone administration. Uses pharmacokinetic principles in patient monitoring and in designing therapy utilizing VA National Clinical Practice Guidelines when appropriate. Acts as a mid-level provider to manage patients primarily through virtual modalities such as Clinical Video Telehealth and VA Video Connect. Will also provide patient care by telephone and face to face encounters as needed. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Monitors patients prospectively for any deviation from the accepted treatment plan, for the efficacy of the plan, and for any adverse reaction or drug interaction occurring during the execution of the plan. Demonstrates advanced knowledge of pharmaceutics, pharmacodynamics and pharmacotherapeutics as determined by clinical review. Reviews, analyzes, and applies literature to specific patient situations. Documents interventions and recommendations via CPRS progress notes and consults, demonstrating the ability to communicate orally and in writing to persuade and influence clinical and management decisions. Written documentation of all patient interactions is required. Evaluates the need for and provides verbal and written medication information to patients or families concerning their medications and disease states. Serves as advisor to the Chief, Telehealth Service and VISN 6 TelePrimary Care Hub on matters related to clinical practice, drug therapy, and drug selection. Assists the area of practice in meeting all of The Joint Commission medication management standards by having an expert understanding of regulatory and quality standards for the area of practice. May evaluate non-formulary and restricted medication requests from assigned providers. Work Schedule:

Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Experience:
Board Certification in Ambulatory Care Pharmacy, Pharmacotherapy, or other advanced certification is highly desired. A sustained record of contribution and commitment to pharmacy, as defined by ASHP. Telehealth experience is highly desired. Grade Determinations:
GS-13 EXPERIENCE - Qualifying experience equivalent of one (1) year at the GS-12 grade level which is directly related to the position to be filled. Experience includes reviewing, interpreting, and verifying medication orders for appropriateness; processing and filling medication orders; interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care; reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems; contacting providers as appropriate; documenting recommendations and interventions; providing refill extensions and partial medication supplies; taking health and medication histories; performing medication reconciliation; providing drug information; assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations; documenting and assessing adverse drug events (ADEs); assisting in medical emergencies; providing oversight of technical staff in all aspects of medication distribution. Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. The work may require regular and recurring physical exertion, involving standing/sitting for prolonged periods of time, lifting/carrying of pharmaceutical supplies or telecommunications equipment weighing up to 40 lbs., and bending/stooping. The work may entail repetitive wrist movement while using various size syringes, vial cap removal, and typing.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
