POSITION SUMMARY
Provides medical case management services with minimal supervision. Accepts case management referrals; maintains assigned caseload (125-150). Assesses patients healthcare needs and identifies community resources. Promotes and coordinates optimum healthcare services, with cost effective solutions.
The Medical Group Care Manager works predominantly within Practice Groups to deal with the increasing complicated chronic disease patient with multiple co-morbid conditions. The MGCM interfaces with physicians, nurses, all departments within the hospital facilities, and community resources to expedite medically appropriate cost-effective care. The MGCM applies clinical expertise and medical appropriateness criteria to ensure appropriate level of care, resource utilization and discharge planning. Serves as a patient advocate and assists in the identification and improvement of service delivery.
JOB REQUIREMENTS
1. Manages the case load of patients with chronic disease under the care the supervision of a physician.
2. For the patient improves healthcare access and promotes client knowledge and behavior change.
3. Facilitates the transition of care from post hospitalization, ER discharges, nursing home or assisted living
settings. To reduce readmissions and prevent visits for care that would better be delivered in health centers or
provider offices.
4. Facilitates the initiation of diagnostic services, treatment planning, and therapeutic treatment while the patient is
enrolled in the program in the practice setting.
5. Collaborates with the Emergency Department physicians, Hospitalists and/or attending physicians to determine
correct level of care at the time of admission or discharge from alternate care setting.
6. Collaborates in the utilization of clinical physician order sets for designated targeted diagnoses to ensure follow
through on work assignments.
7. Consults with the medical staff, nursing staff, and ancillary department staff to eliminate barriers to the efficient
delivery of care.Identifies service delivery problems and potential for effective case management intervention.
8. Develops and implements case management plans to maximize healthcare outcomes and facilitate wellness.
9. Maintains resource manual for case management program.
10. Assist medical group with coordinating care of the uninsured/unassigned patient and facilates referrals into one
of the Bon Secours Providers of Care.
11. Initiates communication with other care managers on admitted and observation patients to facilitate care.
12. Manages a clinical team with primary practice.
13. Assist with managing office clinic if needed.
14. Manages specialty clinics as needed and performs POC testing according to OSHA guidelines.(ie. Coumadin
clinic)
15. Maintains annual competencies for role.
16. Assist with evaluating competencies of clinical staff on the team.
17. Follow-up with patient on all labs, test, and consults to ensure work is done.
18. Documents all communication with patient in electronic medical record.
19. Demonstrates proficient computer skills and is able to function within the electronic medical record.
20. Maintains data on patients outcomes.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
